PrimeCare members understand that patient health is shaped by more than what happens in an exam room—access to food, housing, mental health support, and financial assistance all play a critical role in long-term wellness. This page is designed to help you connect your patients with trusted community organizations offering essential services. FOOD ASSISTANCE

Jackson County’s primary community action agency since 1976. ACCESS operates a network of 30+ food pantries, a Regional Food Bank, and a Mobile Food Pantry program that reaches food-insecure families at clinic sites and high-need neighborhoods throughout the Rogue Valley. Also provides energy assistance, weatherization, rental support, and housing services. 541-779-6691

Emergency food assistance for individuals and families in the Ashland, Talent, and Phoenix area. Provides groceries, connects clients to partner agencies, and maintains an updated directory of additional food resources throughout Jackson County. 541-488-9544

A regional food bank serving all of Josephine County through a network of 9 pantries, 5 community kitchens, and 11 supplemental agencies. Each month, they provide meals to over 13,000 people — including working families, seniors, children, and individuals experiencing homelessness. HOUSING AND SHELTER

Jackson County’s only walk-in resource center for unhoused and low-income individuals. OHRA offers a 24/7 year-round shelter, laundry and shower services, and on-site Resource Navigators who help clients with housing, benefits enrollment, employment, utility assistance, ID documents, and referrals to partner agencies. 541-631-2235

Provides shelter, food, and transitional housing programs for individuals experiencing homelessness in Southern Oregon. Focused on stabilization first, then a pathway to permanent housing through case management, job training, and connections to medical and social services. 541-499-0880

MENTAL HEALTH AND ADDICTION

County-operated outpatient mental health services for adults, including therapy, case management, peer support, and an Assertive Community Treatment (ACT) program. Primarily serves uninsured individuals, OHP members, Medicare, and some private insurance. 24/7 crisis line: 988 or 541-774-8201

A comprehensive behavioral health provider serving all ages across Jackson and Josephine counties. Offers mental health therapy, addiction counseling, crisis intervention, peer support, housing services, and community integration programs. Accepts OHP, Medicare, and sliding scale fees. Southern Oregon’s volunteer-led affiliate of the National Alliance on Mental Illness provides free mental health education, support groups, and community presentations through their Medford Resource Library. MEDICARE AND MEDICAID ASSISTANCE

Free, confidential Medicare counseling from trained, unbiased volunteers. SHIBA helps patients understand Medicare options, compare plans, navigate enrollment, and identify low-income assistance programs. Available statewide through the Oregon Department of Human Services. COMMUNITY RESOURCES

Provides crisis intervention, domestic violence advocacy, and the HelpLine (541-779-4357) connecting Jackson County residents to emergency services, shelter referrals, and community support.

Jackson Youth System of Care is a network of accessible, effective, community-based services available to young people in Jackson County. This network builds meaningful partnerships with families and youth by addressing their cultural and linguistic needs in order to help them thrive at home, in school, in the community, and throughout their lives.

A nonprofit community health center founded in 1972, with locations in Medford, Ashland, White City, and beyond. Provides primary care, dental, behavioral health, prenatal care, pharmacy services, and substance use treatment—all on a sliding fee scale.
